have in a waterproof zip lock bag-

-Health records (vaccinations, allergies,
medications) for each animal

Microchip Company, phone number and chip number for each animal

a flyer for each of your pets-

-Take a photo of your family and
your pet together and one of just your

of the flyer; add both photos,
your information,
